What is ECFMG certification and what are the steps?

ECFMG certification is the credential the ACGME requires before an international medical graduate can begin an accredited residency or fellowship. ECFMG calls it "the standard for evaluating the qualifications of international medical graduates (IMGs) entering the U.S. health care system." It is also one of the eligibility requirements to take USMLE Step 3, and U.S. licensing authorities require it for an unrestricted license.

The mechanic that is easy to miss sits at the end: a certificate earned through a Pathway carries an expiration date. First, the steps, in ECFMG's order.

ECFMG defines IMGs as individuals who received their basic medical degree from a medical school located outside the United States. "The location of the medical school, not the citizenship of the individual, is what determines whether they are IMGs." Graduates of Canadian medical schools on or after July 1, 2025 are considered IMGs for entry into U.S. GME; earlier Canadian graduates are not IMGs and are not eligible for ECFMG certification.

The school must be listed in the World Directory of Medical Schools with an ECFMG Sponsor Note. "If there is no ECFMG Sponsor Note on the Sponsor Notes tab of your medical school's listing, you are not eligible for ECFMG Certification." The credential rule underneath is at least four credit years, meaning academic years credited toward the medical curriculum, from a school that carries the note.

"You must establish a MyIntealth account and meet our identity verification requirements before you can begin the Application for ECFMG Certification." The application "confirms their intent to pursue ECFMG Certification and collects detailed information about their medical education." On the current fee page, account establishment is $110 and the Application for ECFMG Certification is $580.

The medical science examination requirement is two exams: "you must pass Step 1 and Step 2 Clinical Knowledge (CK) of the United States Medical Licensing Examination (USMLE)."

There is a clock. "ECFMG requires that IMGs satisfy the examination requirements for ECFMG Certification within a seven-year period," which "begins on the date of the first exam passed and ends exactly seven years from that date." Miss it and "your earliest passing performance will no longer be valid for ECFMG Certification." ECFMG "will not notify you of upcoming deadlines to meet the seven-year requirement." An accepted Pathway must also land inside that same seven years.

Until 2020, Step 2 CS covered this. Now applicants satisfy both requirements by meeting an ECFMG Pathway, "which includes attaining a satisfactory score on the Occupational English Test (OET) Medicine." Anyone who still holds a valid Step 2 CS pass does not have to, and is not eligible to, pursue a Pathway.

The 2027 Pathways come in six versions, numbered Pathway 1 through Pathway 6. Pathway 1 is "for applicants who currently hold or have recently held a license/registration to practice medicine without supervision." Pathway 6 is "Evaluation of Clinical Patient Encounters by Licensed Physicians." One rule cuts across them: applicants who failed Step 2 CS one or more times "must apply to Pathway 6," regardless of eligibility for other Pathways.

The OET requirement has no carve-outs: "All Pathways applicants, regardless of native language, language of instruction at medical school, or citizenship, must satisfy this requirement. There will be no exceptions." The minimum is 350 on Listening, Reading, and Speaking and 300 on Writing, in a single test administration; fall short on one sub-test and all four are retaken. Scores must date from on or after January 1, 2025. The Pathway application fee is $945. ECFMG announced on May 15, 2026 that the 2027 application was available; the Pathways application deadline for 2027 Match participation is January 31, 2027, Eastern Time, and ECFMG suggests sitting OET "on or before the last available test date in December 2026."

"IMGs must document the completion of all requirements for, and receipt of, the final medical diploma." ECFMG "verifies every IMG's final medical diploma with the appropriate officials of the medical school that issued the diploma," and asks the school for the final transcript. Nothing counts until the school sends those directly to ECFMG. Verification is $220 on the fee page, $110 each for diploma and transcript.

Certificates are issued "approximately two weeks after all of these requirements have been met," and they carry a "valid through date, if applicable."

ECFMG's own Match page says: "You do not need to be ECFMG Certified in order to participate in the Match." The Match requires the examination requirements to be met, with results available, by the NRMP Rank Order List Certification deadline, including an accepted, unexpired Pathway at that time. Certification is required to start: "You must have a valid, unexpired ECFMG Certificate to enter a U.S. graduate medical education (GME) training program accredited by the Accreditation Council for Graduate Medical Education (ACGME)."

"If you are certified by ECFMG based on a Pathway, your ECFMG Certificate will expire when your Pathway expires." For 2027 Pathways, that date is December 31, 2029. Certificates based on the former Step 2 CS are "not subject to expiration."

Three routes make a Pathways certificate valid indefinitely: "successfully complete at least 12 months of clinical education in a U.S. GME training program accredited by" the ACGME, "successfully complete at least 12 months of clinical education in a U.S. non-standard training program associated with an ACGME-accredited program," or "hold an unrestricted license to practice medicine in a U.S. state, U.S. territory, or the District of Columbia." J-1 physicians sponsored by Intealth get this automatically after 12 months. ECFMG sends a confirmation letter; "You will not receive a new ECFMG Certificate." A Pathway that expires first can be revalidated, but only through Pathway 1 or Pathway 6.

ECFMG is "the designated Dean's office for students and graduates of international medical schools" in ERAS. IMGs begin by requesting an ERAS Token through MyIntealth. The fee is $185, non-refundable. "You need one Token per ERAS season, and you can use that one Token to apply to as many specialties and programs as you want during the ERAS season." Tokens for ERAS 2027 became available June 24, 2026. ECFMG recommends submitting the MSPE and transcript "no later than September 11." The ECFMG Status Report, which "confirms your ECFMG Certification status" and shows exam months and years but not USMLE scores, "will be transmitted automatically to your MyERAS application and will update automatically."

Which Pathway fits, and when to sit OET, are questions for your dean's office or advisor. ECFMG's Information Booklet is where the rules live: applicants pursuing certification "must become familiar with and will be subject to the policies and procedures detailed therein."
