How much does it cost to apply to medical school?

"How much does it cost?" has two honest answers. One is a list of line items. The other is a timing rule that decides whether the AAMC's fee waiver applies to you at all. This page gives you both, in that order.

For the 2027 cycle the AAMC states the fee plainly: "The application processing fee is $180 and includes one medical school designation. Additional school designations are $48 each." Tax, where applicable, is added at checkout. AMCAS "will not accept any offline payments, including checks, money orders, or credit card information sent via mail, fax, email, or phone."

So the bill scales with your list. One school is $180. Ten schools is $180 plus nine times $48, or $612. Twenty schools is $1,092, the figure the AAMC itself uses to describe its fee waiver.

Two list rules affect the bill. Once you submit, "you can't remove or substitute schools on your designation list under any circumstances." You can add schools afterward if their deadlines have not passed, and "There is a fee for applying to additional medical schools." Refunds are narrow: you are eligible only if you withdraw "before your application begins the AMCAS verification process," and even then the refund is "only for those schools for which the published deadline has not yet passed." If no published deadlines have passed at the time you withdraw, you get "a full refund minus a $180 non-refundable service fee."

AMCAS is not the whole cost. The AAMC's own cost page says "Most medical schools require a secondary application. Those fees range in cost." This page stays with the numbers the AAMC publishes.

For all 2026 test dates the standard registration fee is $355. Examinees testing outside the U.S., Canada, or U.S. territories pay a $130 international fee on top. Rescheduling runs from $55 at 60 or more days before the exam to $210 at 10 to 29 days. Cancel 30 or more days out and $175 comes back. Cancel 10 to 29 days out and the refund is $0.

The AAMC Fee Assistance Program is the lever on all of this. A 2026 award includes:

Three tests. You must have "a U.S.-based home address." You must be preparing to apply, meaning not currently accepted or enrolled in any medical school, not a graduate of one, and not preparing to apply for residency. And income: each household on the application, "yours and your parents, if applicable," must have "a 2025 total family income that is equal to or less than 400% of the 2025 national poverty level for your family size." The AAMC posts the resulting thresholds. For the 48 contiguous states and D.C., a family of one is $62,600, two is $84,600, three is $106,600, and four is $128,600. Alaska and Hawaii run higher.

The parent rule is where applicants stumble. If you are under 26 at submission, the program "requires your parents' financial information," and requires it "regardless of your marital status, your tax filing status (independent or dependent), your parents' country of residence, or whether your parents are willing to provide documentation." The AAMC says these requirements "are different from federal aid programs such as the Free Application for Federal Student Aid (FAFSA)."

Documents: your "Most recent federal tax return (IRS Form 1040), if filed," W-2s or 1099s if applicable, and parents' tax information when your age requires it. Signed pages need "a handwritten or verified digital signature." Typed signatures are not accepted.

Here is the line that decides whether any of the above applies to you. From the 2027 AMCAS Applicant Guide: "If you plan to apply to the program, you must submit a completed Fee Assistance Program application before you submit your AMCAS application."

That is not a courtesy. "Fee Assistance Program benefits are not retroactive." The Essentials guide is explicit: "Do not submit your AMCAS application, register for the MCAT exam, or purchase MCAT Official Prep subscription products or a Medical School Admission Requirements subscription before you receive the outcome of your Fee Assistance Program application." And: "No refunds will be issued for any fees you paid before your Fee Assistance Program application was approved."

The clock: once every document is in, the staff's stated goal is to "contact you regarding their findings within ten business days." If something is missing or unsigned, the file drops to "Submitted and On Hold," and the verification line only starts "Once all required documents are received." The AAMC adds that it "is not responsible for any MCAT or PREview registration deadlines or medical school application deadlines you might miss" during review.

Put the dates side by side. The 2027 AMCAS application opened May 5, 2026. A 2026 award, approved on any date in 2026, expires Dec. 31, 2027. The back end is generous. The front end is the trap: ten business days, plus any hold, and the submit button waits for it.

Once approved, "you may not change or update your application for any reason." Benefits begin on the approval date. You may apply "once per calendar year" and be awarded "a maximum of five (5) times during your lifetime." One rule for repeat applicants: "Unused AMCAS benefits from a previous approval are forfeited when your new award is approved."
